What are androgens and what role do they play in the female body?

Androgens are often described as male sex hormones, but they also play an important role in women. Their effect depends on concentration. At physiological levels, androgens support libido, energy levels, muscle mass, bone health, and metabolic processes.

What is androgen excess and how does it lead to hormonal acne?

Androgen excess means that androgen levels in the female body are elevated – either absolutely, or relatively due to low estrogen. This can be temporary, for instance after stopping hormonal contraception, or caused by chronic stress that disrupts hormone balance. Genetic factors also play a role, including in Polycystic Ovary Syndrome (PCOS). In addition, excess weight and insulin resistance can increase androgen production. These causes should be medically evaluated.

Androgens, especially the metabolite DHT, act on hair follicles and sebaceous glands. Too much DHT can shrink hair follicles and shorten the hair growth cycle, leading to hair loss (androgenetic alopecia). At the same time, DHT can stimulate unwanted hair growth on androgen-sensitive areas (e.g. chest, back, chin).

In the skin, androgens stimulate sebum production and change sebum composition. This can clog pores, trigger inflammation, and cause breakouts – this is known as hormonal acne.

What is the androgen rebound?

Androgen rebound can occur when androgen production has been suppressed for a longer period, such as through hormonal birth control or medication. After stopping, some women experience a temporary surge in androgen activity. This commonly appears around three months after discontinuing hormonal contraception, as part of the post-pill syndrome. Typical symptoms include hormonal acne, breakouts, oily skin, or androgen-related hair loss.
